Someone was afflicted with a disease, likely tertian fever (i.e., malaria). For treatment, he had to drink 6 glasses of a very bitter medicine (probably made from cinchona bark) daily to prevent dangerous attacks. If such an attack occurred, he had to consume various types of drinks to avert the risk to his life. The question arises whether, in light of this, drinking the medicine would be permissible on Yom Kippur from the outset. David ben Jacob de Mesa grants permission. This is the first dating in this book that indicates the year. The 5 of the year is highlighted by a particularly large typeface.
