Question Full A woman from the city of Pozen named Sara bat Jacob has a civil divorce. She comes to Amsterdam and tells that her husband Theodor Levy went to New York and there married another woman. She asks the head of the Bet Din in Amsterdam for help in getting her husband to give her a Get so she can remarry. The head of the Bet Din then wrote to a friend rabbi in New York about this matter and to intercede on behalf of this woman. And after a few months a messenger arrives from New York and in his hand there is a Get with the appropriate credentials, and also a letter from the friend rabbi in New York writing that he had difficulty finding the husband, as there are five people in New York with the name Theodor Levy. In the end one of them admitted that he had a wife in Pozen whose name was Sara bat Jacob, and nevertheless he married another woman. After pleading with him the man agreed this time to issue the Get. He said that his Hebrew name is Tuvyah ben Abraham Halevi. In the letter it states "behold in the Get and credentials the man is only named Tuvya ben Abraham Halevi". Yet the woman never knew the man's Hebrew name, and is only known to her as Theodor Levy. Now the question is if the Get is kosher and should be given to the woman.
